We are excited to introduce our Online Patient Registration!  We are asking new and previous patients to fill this out one time as we transition into Electronic Health Records.  You can fill out your paperwork online and submit it to us directly through a secured portal.  You won't have to worry about printing and filling out your paperwork to bring to your appointment.  Not to mention the time you will save in the office!  This will update all your personal information, medical history and insurance information.  That way when you show up to your appointment you are ready to go!
